Gift Aid Form Procedure

Purpose:
The Gift Aid Form needs to be completed at the beginning of a Student’s first therapy session. See page 2.

Procedure:
The therapist or trainee should ask the Student if they earn enough to pay tax and/or give Gift Aid in other situations.

If the Student does not pay enough tax or seems uncomfortable, then the Gift Aid Form should be put to one side.

If the Student agrees then they should complete all the sections of the Gift Aid Form.

The therapist or trainee will then thank them very much for their additional support of Ayurva.

Once completed this form should then be passed on to the Office Manager who will then file it in the secure Student’s file.
